Family Case Management (FCM) is a case management program founded by the Illinois Department of Human Services.  Its goal is to promote wellness and healthy lifestyles in income-eligible pregnant women, infants, and children by assisting with early access to needed health care and by providing health education and other services. Temporary medical cards are provided to income eligible pregnant women to enable them to access prenatal care as early as possible. Services are offered to meet medical, social, emotional, nutritional, environmental, safety, financial, educational, and vocational needs. Follow-up, referrals, advocacy, home visits, parenting tips and assistance are provided at no charge throughout pregnancy and for a year after the infant's birth. For more information, call ext.110.

Teen Parent Services (TPS) is an intensive case management program funded by the Illinois Department of Human Services. The TPS program provides comprehensive services to pregnant or parenting teens, under age 21, who have not completed secondary school. The goal of TPS is to increase the number of pregnant or parenting teens who graduate from high school or receive their GED, reduce subsequent births, and promote wellness and healthy lifestyles for themselves and their children. HealthWorks is a medical case management program to ensure that children in the care and custody of DCFS have ongoing and timely access to essential health care. JCHD is the lead agency providing services to Franklin, Gallatin, Jackson, Perry, Saline, White, and Williamson Counties. For more information, call ext. 164.

Women, Infants, and Children (WIC) is a supplemental nutrition program which offers services to pregnant women, breastfeeding and postpartum women, infants and children up to 5 years of age. Participants must meet income eligibility requirements and be found to have a medical or dietary health risk. The services provided by WIC include physical assessments, nutrition education, counseling and assessment, and supplemental food packages. For more information, call ext. 148.

JCHD is an All Kids agent. Eligible families can obtain All Kids insurance by applying at WIC. Appointments are required. For more information, call ext. 148.
